<p><h3>Overview</h3><p>The Registered Nurse (RN) role models clinical excellence and the core competencies of the College of Nurses of Ontario (CNO), as well as the policies and procedures of Southlake Regional Health Centre (Southlake). The RN works collaboratively as a member of the health care team and provides direct patient care based on the three-factor framework: The Client, Nurse and the Environment. These factors impact decision making related to care-provider assignment as well as the need for consultation and collaboration among care providers. All nurses commit to lifelong learning.</p>
<p>The successful candidate will be motivated, self-directed and a key member of a dynamic interdisciplinary team with defined roles focused on quality patient care and patient flow. The successful candidate will be responsible to provide excellent care to a diverse acute medical patient population in a fast-paced setting.</p>
<p>The Clinical Nurse Consultant (CNC) is a unique and advanced position that involves working within community-based inter-professional Palliative Care teams to ensure the delivery of effective, high quality care to patients living with a life-threatening illness and their families in their home setting.</p>
<p>The CNC will provide expert mentorship and support to primary clinicians, nurses, and other members of the health care team to enhance palliative care knowledge and capacity within the community.</p>
<p>As Lead Educators for Hospice Palliative Care in the community, the CNC will provide accredited education courses to health care professionals across the Ontario Health Central Region.</p>
<p>As a member of HPC Teams, the CNC will participate in the provision of a 24/7 on call consultation service with an appropriate level of physician back up support.</p>
<h3>Responsibilities</h3>
<ul>
<li>Provide direct patient care to patients in a fast-paced, diverse acute medical setting.</li>
<li>Collaborate with an interdisciplinary team and adhere to CNO standards and Southlake policies.</li>
<li>Mentor and support primary clinicians, nurses, and other care team members to enhance palliative care knowledge and capacity within the community.</li>
<li>Lead and deliver accredited education courses as Lead Educator for Hospice Palliative Care in the community.</li>
<li>Participate in a 24/7 on‑call consultation service with physician backup support as part of HPC Teams.</li>
</ul>
<h3>Qualifications and experience</h3>
<ul>
<li>Motivated, self-directed, and a key member of a dynamic interprofessional team focused on quality patient care and patient flow.</li>
<li>Ability to provide excellent care to a diverse acute medical patient population in a fast-paced setting.</li>
<li>Experience within community-based inter-professional Palliative Care teams is desirable.</li>
