This position is ideal for therapists who value flexibility, autonomy, and meaningful client relationships within a community-based model. Please note, this position will require <strong>in-person home visits </strong>with vulnerable community members. </p><p>While we thank all interested applicants, we are only able to contact those who we are inviting to an interview. </p><p></p><p><strong>Key Responsibilities: </strong></p><ul><li>Complete <strong>initial home-based physiotherapy assessments within 24 hours</strong> of referral </li><li> Provide follow-up sessions focused on mobility, gait training, strength/balance retraining, pain management, and functional progression </li><li> Develop, implement, and adjust <strong>individualized treatment plans</strong> in alignment with client goals </li><li> Educate clients and caregivers on home exercise programs, safety strategies, functional mobility, and equipment use </li><li> Identify and address <strong>fall risks, environmental barriers, and safety issues</strong> in the home </li><li> Collaborate with <strong>OTs, PSWs, nurses, physicians, and HCCSS/OH@H care coordinators</strong> to support coordinated care </li><li> Complete documentation and charting accurately and in a timely manner according to community-care standards </li><li> Maintain ongoing communication with the office regarding availability, scheduling updates, and urgent visit capacity </li><li> Uphold The Care Company’s commitment to <strong>safe, ethical, and compassionate community care</strong></li></ul><p><strong>Requirements</strong></p><p><strong>Scheduling: </strong></p><p><strong>Flexible part-time schedule (Monday–Friday)</strong></p><p><strong>Occasional Saturday/Sunday availability</strong> based on client needs </p><p>Must be able to complete an <strong>initial in-person home assessment within 24 hours</strong> of receiving a visit request </p><p>Clients are located across <strong>Toronto and the Greater Toronto Area</strong>, including but not limited to: </p><ul><li> Downtown Toronto, Midtown, East/West Toronto </li><li> North York, Richmond Hill, Markham </li><li> Mississauga, Brampton, Vaughan, Woodbridge</li><li> Pickering, Ajax, Whitby, Oshawa, Bowmanville and select areas of Durham region </li></ul><p>Ability to independently manage scheduling, respond promptly to referrals, and coordinate visit times directly with clients </p><p><strong>Travel may be required between client homes</strong></p><p></p><p><strong>Qualification and Experience Requirements: </strong></p><p><strong>Home and community care experience is a MUST.
