In every moment. </p><p>That’s The Y Way. </p><p></p><p><strong>Position Summary:</strong></p><p>The Follow-Up Support Worker provides intensive case management services to individuals who have recently transitioned into housing through the Housing First program. This role focuses on maintaining housing stability, improving the overall quality of life, and supporting participants in achieving their personal goals.</p><p><strong>Key Responsibilities:</strong></p><ul><li>Provide client-centered, trauma-informed follow-up support to individuals housed through the Housing First program.</li><li>Conduct regular home visits and wellness checks to monitor housing stability and support client success.</li><li>Develop and implement individualized service plans in collaboration with program participants.</li><li>Connect clients to community resources and services, including mental health, addictions, primary care, employment, and income supports.</li><li>Advocate on behalf of clients with landlords, service providers, and systems to ensure access and equity.</li><li>Maintain accurate and timely documentation in compliance with agency policies and funder requirements.</li><li>Participate in team meetings, case conferences, and professional development activities.</li></ul><p><strong>Qualifications:</strong></p><ul><li>Bachelor’s degree in social work, Human Services, or a related field (or equivalent experience).</li><li>Minimum 2 years of experience working with individuals experiencing homelessness, mental health issues, addictions, or other barriers to housing.</li><li>Knowledge of the Housing First model, harm reduction, and trauma-informed care principles.</li><li>Strong interpersonal, crisis intervention, and problem-solving skills.</li><li>Ability to work independently and collaboratively in a team environment.</li><li>Valid driver’s license and access to a reliable vehicle.</li><li>Current First Aid/CPR and a clear Criminal Record Check with Vulnerable Sector Screening.</li></ul><p><strong>Working Conditions:</strong></p><ul><li>Frequent community-based work and travel within the region.</li><li>Hybrid work environment combined with home visits and community outreach.</li></ul><p></p><p><strong>What the Y Brings You</strong></p><ul><li>A meaningful career with opportunities to grow while building your community. <br><br></li><li>A culture of honesty, respect and pride in what we do. <br><br></li><li>Colleagues who bring diverse perspectives and talents to the workplace. <br><br></li><li>A comprehensive benefits package. <br><br></li></ul><ul><ul><li>Membership to our health, fitness & aquatics facilities for you and your family <br><br></li><li>Reduced fees in YMCA programs such as childcare and day camps (depending on availability and employment status). </li></ul></ul><p></p><p><em> </em></p><p><em>The YMCA of Northern Alberta strives to integrate our values of diversity and social inclusion in all that we do.
