They provide integrated care, offering a wide range of services such as primary care, mental health services, addiction care and employment supports. Availability between 8am-9pm Monday-Friday, occasional weekend shifts between 8am-8pm. Note, this posting is for an active vacancy.<br />Salary: $61,201.01 - $72,001.19 annually. (Candidate qualifications will be considered when determining the starting salary)<br />Location:This position is based out of 30 Centre St, Orangeville. Due to the nature of this program, this role is on-site and/or community based.</p><p><br /></p><p>What You Will Do:</p><ul><li>Collaborate in developing and implementing individualized wellness plans with clients</li><li>Use standardized assessment tools designed to improve and/or stabilize clients physical and mental health</li><li>Offer a strength-based approach that recognizes clients unique needs</li><li>Provision of case management services including but not limited to assist with activities of daily living, conflict resolution, and crisis avoidance and mental health counselling</li><li>Provide comprehensive, holistic, trauma-informed and person/family centered access to services that address the social determinants of health</li><li>Partner with clients and their families as members of the health care team</li><li>Develop and maintain a community resource network</li><li>Refer to partnering organizations and provide linkages to community supports</li><li>Work independently and collaboratively within a multidisciplinary team</li><li>Understand and be able to integrate a philosophy of person and family centered care into daily responsibilities</li><li>Demonstrate a strong commitment to quality care and service delivery based on the HART Hub Mission and Values<br /></li></ul><ul><li>Facilitate groups, including the creation of new groups as needed and acting as a back-up facilitator</li></ul><p><br /></p><p><em>This posting description indicates the key responsibilities and core functions of the position. It is not designed to cover or contain a comprehensive listing of activities, duties, or responsibilities.</em></p><p><br /></p><p>What You Need to Join Our Team:</p><ul><li>Post-secondary education in Social Work, Health Services, Psychology or equivalent</li><li>Minimum 2 years experience providing Mental Health case management or equivalent care</li><li>Demonstrated knowledge of serious mental illness, concurrent disorders, and/or dual diagnosis, and experience with assessment</li><li>Experience working within the homelessness sector</li><li>Knowledge of evidence-based practice for mental health services<ul></ul></li><li>Strong interpersonal skills with proven ability to approach, listen, build trust, and communicate across diverse cultures and intersectionality.
